There is also a quality change that the scan cannot see.

Thank you for spelling out the reasoning rather than just the conclusion. Taking it to my next appointment.

Adding the clinical framing, because it changes how the question reads.

Bone density monitoring on lean mass: DEXA bone density at baseline and month 12.

Lumbar spine T-score: -0.10 → -0.7 (stable to slightly improved)
Femoral neck T-score: -0.12 → -0.8 (stable)

No significant bone loss despite 77 lbs of weight loss. Key factors: adequate calcium (1200mg/day), vitamin D (5000 IU/day), resistance training 3x/week, and adequate protein. Weight-bearing exercise is critical for maintaining bone density during rapid weight loss.

Body composition tracking via bioimpedance on lean mass (InBody scan every 3 months): Metric Baseline Month 3 Month 6 Month 9 Skeletal Muscle (lbs) 75…

Back pain as a lean mass side effect: not from the medication itself, but from rapid body composition changes. Losing 47 lbs shifts your center of gravity and your core muscles need time to adjust.

Physical therapy has been essential. Core strengthening exercises (planks, dead bugs, bird dogs) daily for 10 minutes have eliminated my lower back pain. Your body is literally relearning how to carry itself.

The intervention that works is boring and well evidenced: resistance training two to three times a week at a genuine effort, protein at 1.4 to 2.0…

Sharing my comprehensive lab data relevant to lean mass:

BiomarkerBaselineMonth 3Month 6Month 9
Weight (lbs)275240235195
A1C (%)7.56.55.65.2
hsCRP (mg/L)5.04.02.51.1
Triglycerides225165135115

Protocol: Tirzepatide 15mg/wk, resistance training 4x/week, protein 1.6g/kg/day.

The improvements in lean mass are clearly reflected in these numbers. The inflammatory marker reduction (hsCRP) is particularly striking.

Moderator note: good thread. Keeping it here rather than moving it, because the question is general enough to be useful. No action needed from anybody.
