6 month update on cardiovascular risk: down 42 lbs, off blood pressure meds, A1C normalized.

This is where I part company with the consensus forming above. The "earlier than weight loss explains" argument is weaker than this thread makes it sound. Blood pressure and inflammatory markers move fast and are downstream of early weight loss, so the mechanism is not as cleanly separable as the summaries imply.

Correct me if the detail matters more than I have assumed.

Prescribed on cardiovascular grounds rather than for weight, and almost everything written for patients assumes the opposite.

Senior perspective on cardiovascular risk: I'm 67 years old and started this journey skeptically. My endocrinologist recommended it after years of failed interventions.

15 months later: down 50 lbs, more mobile, pain reduced, medications simplified. My quality of life has improved dramatically. I wish this existed 20 years ago.

To other older adults hesitating: the SELECT trial proved benefit in our age group. You deserve to feel good in your body regardless of age.

Metabolic syndrome resolution on cardiovascular risk: I went from meeting 4 of 5 diagnostic criteria to meeting ZERO after 9 months of treatment.

The 5 criteria (and my journey):

  1. Waist circumference: 48" → 34" ✅ Resolved
  2. Triglycerides: 244 → 109 ✅ Resolved
  3. HDL: 32 → 56 ✅ Resolved
  4. Blood pressure: 144/94 → 122/76 ✅ Resolved
  5. Fasting glucose: 124 → 84 ✅ Resolved

Metabolic syndrome reversal is, in my view, the most medically significant outcome of GLP-1 therapy.

Following on from tom_AK — and this may be the naive question:

What would you measure differently if you were starting again?

...we don't know the long-term effects of cardiovascular risk...

This is a fair point, and I think intellectual honesty requires acknowledging it. GLP-1 agonists in their current form have ~8-10 years of human exposure data. That's not nothing, but it's not 30+ years either.

However: the risk-benefit calculation should also consider the KNOWN long-term effects of untreated obesity — diabetes, cardiovascular disease, cancer, joint destruction, reduced lifespan by 5-10 years.

Uncertainty about GLP-1 long-term safety vs certainty about obesity consequences. The calculus seems clear to me, but reasonable people can disagree.
