Something nobody's mentioned — the cardiometabolic secondary endpoints were also meaningfully different:

MarkerTirz changeSema change
Waist circumference-17.8 cm-12.4 cm
Triglycerides-32.4%-21.6%
HDL-C+8.7%+4.3%
Systolic BP-7.2 mmHg-5.1 mmHg
hsCRP-58.3%-42.1%

The triglyceride and HDL improvements are really noteworthy and likely GIP-mediated. GIP receptors on adipocytes play a role in lipid storage and mobilization that GLP-1 alone doesn't replicate.

Also the hsCRP reduction suggests a more robust anti-inflammatory effect, which is consistent with the SURMOUNT-OSA and cardiovascular sub-studies.

Ok those cardiometabolic numbers are admittedly impressive. The triglyceride difference is hard to argue with.

I stand by my point about individual variation though. My sister lost 22% on Wegovy alone. I'm at 19% on tirz 12.5mg. Genetics are wild 🤷‍♀️

Totally fair ingrid_STO. The best GLP-1 RA is the one that works for YOU, that you can access, and that you can tolerate. Population-level data informs clinical decision-making but individual responses vary enormously.

What I find most exciting is that we now have TWO highly effective options with head-to-head data. Competition drives innovation and (eventually) price reduction. The real winners here are patients.

Short version: On average, tirzepatide (Mounjaro/Zepbound) produces more weight loss than semaglutide (Ozempic/Wegovy). But "on average" is doing heavy lifting in that sentence. Both are very effective. Talk to your doctor about which one your insurance covers, what side effects you're most concerned about, and whether you have other conditions (like high triglycerides) that might favor one over the other.

You genuinely cannot go wrong with either one. Congrats on starting the journey!
