Pharmacist here. The reason is FDA indication and NDC codes. Even though tirzepatide is the same compound:
- Mounjaro (NDC 00002-1504-xx) — FDA approved for type 2 diabetes, billed under diabetes drug class
- Zepbound (NDC 00002-2355-xx) — FDA approved for chronic weight management, billed under anti-obesity drug class
Insurers set coverage by drug class. Most plans cover diabetes medications broadly but explicitly exclude anti-obesity medications. It's the same molecule but legally, regulatorily, and from a billing perspective, they are different products.
It's frustrating but that's how the system works.
